Athletic Insurance Information
Dear Parents:
The coverage for injuries arising from UIL activities will be provided by two policies; an allinterscholastic sports plan and a high limit catastrophic plan. The sports plan provides up to $25,000 for "covered expenses" after the $250 deductible is paid by the parents and after any personal or group medical insurance on the athlete has paid its benefit if available. The high limit catastrophic plan covers injuries in excess of $25,000 up to $5,000,000 of "covered expenses" in excess of those paid by personal or group medical insurance. Catastrophic coverage includes athletics and cheerleading.

Certain "covered expenses" are limited within the policy. Filing of Claims:

Claim forms must be submitted within 90 days of the accident.

Treatment must begin within 90 days of the accident.

Only one claim form per accident is required.

The athletic supervisor (coach or trainer) or activity sponsor completes the top portion of the form and the parent must complete the bottom portion of the form.

Claims can only be processed with Itemized bills; statements are not acceptable.

If parents have other insurance, they must file with their primary carrier, and then submit copies of the Itemized Bills and EOB's to Macori Administration.
NOTE: The insurance company cannot request itemized bills from providers unless the parent has given permission to the doctor or hospital for release of this information. 7. The average length of time required to adjudicate a clean claim is 10 days. For all other claims, processing time is 30 to 60 days, depending on the response time for requested information needed to process the claim.
